Tissue collection registration

The Human Tissue Resource Centre is trying to identify all tissue collections/tissue banks currently held, and log them on one central register. This register will be an essential part of the process of maintaining the licence granted by the Human Tissue Authority. 

The Registration and Listing form can be downloaded below and completed electronically. Check boxes can be marked by using the mouse and clicking on the box, and text can be entered in all fields’ shaded grey. Please print and sign the completed form. It is recommended that a copy of the registration and listing form is kept for your records, and to update us with any changes to information.

Tissue collections which were present prior to the Act still need to be stored under licence and the HTRC should be informed.
