Requesting your maternity notes

If you are receiving care at Barts and The London Maternity Service and wish to obtain copies of your health records (including your maternity notes), please contact our health records team on 0207 377 7000 and ask for extension 2356 (or ask for the health records team) to make this request. You will then be sent an 'application for personal information' form to complete and return to the health records team, which will be processed and responded to.
