Barts and The London Endocrinology Centre
Our patients' needs are complex and their endocrine disorders are often managed by doctors who each work in separate departments, so each of them will focus on different aspects of treatment. Our transition clinics are aimed at ensuring a smooth transfer between the children’s and adult services and all of our patients who will need long term endocrinology follow-up treatment are referred to it.
Patients will attend our transition clinic when they are around 16-17 years old where we will see them two to three times in the adult clinic setting before formally introducing them to our adult endocrinology consultants. We will introduce the patient and their parents to the adult consultant at the last transition appointment. During this crucial stage our children’s and adult endocrine specialists will work closely together to keep patient care continuous.
